You can check your Unum claim status and manage your claim on the web at unum.com/claims, or you can use the MyUnum for Members mobile app.

The easiest way to file a claim or leave with Unum is online or with the mobile app. If your claim is for Disability Insurance benefits, check with your HR department at work to find out whether you can file over the phone. For all other benefits, you can call 1-800-635-5597 to file your claim.

Yes! If you cannot file online, you can get a claim form at unum.com/claims or contact your HR department at work. Follow the instructions on the form to mail or fax your completed form, but for fastest results, file online if you can.

When filing a claim or leave, you may be asked for

Your name and address, your employer’s information, your job title and your regularly scheduled work hours. For a leave request, you may need to tell us about your absences from work and expected return-to-work date.

Two-step verification is an online data protection method that creates an additional step to help protect your account information, even if your login ID and password have been compromised.

E-consent gathers your permission to transfer information electronically so that we can provide documents to you via our secure website or by email. Without your affirmative e-consent, we can only send you documents via the U.S. Postal Service or fax, which may cause delays. Likewise, you may only send us documents in the same manner.

Register and log in to the MyUnum for Members online portal to see your personal dashboard, update your claim and stay informed. For the fastest service, sign up for text or email alerts so that you will know right away if more information is needed — no waiting for a mailed letter.

When you file online, you can finish a claim in less than 20 minutes. If you can’t finish in one sitting, the guided claim process will automatically save your progress. You can always find this claim on your home dashboard and finish it at your convenience.

If you have medical records, receipts or documents to submit, you can photograph and upload documents with your phone.

Sign up for direct deposit when you file online, and you can receive benefits in your account up to a week faster than by paper check.

Our guided claim flow will ask questions to determine all benefits that you may be eligible to receive. This means all applicable claims or leaves will be filed, and you only have to submit the form once.

For example, if you are filing an Accident claim, and you also have Hospital coverage, a Hospital claim may also be initiated, if applicable.
